Psychosocial Recovery Coaching
City to Country Care provides the following supports that can be included in your plan under the category of Support Coordination:
1. Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
How can we do this?
City to Country Care will work with you and your support network to assist you in exploring your options, educating you on how to navigate the NDIS and how to effectively use your funding by:
Step 1
Implementing your Plan
We will get to understand who you are, what is important to you and what you need. We will explain your NDIS Plan and support budgets you have been given, we will assist with engaging new services, arrange assessments required, organise service agreements; a Support Coordinator is your go to person for all things relating to your NDIS Plan.
Step 2.
Monitoring and Reporting
It is our role to make sure you are happy with the services you are receiving and if not to look at solutions or alternatives to resolve this. Throughout the duration of your plan we will observe this with you and if required, make necessary changes to services.
We are responsible for reporting progress of your plan on a regular basis to the NDIS. We will complete all reports for the annual review, or if there is a change of circumstances, we will complete all necessary paperwork and reports for this as well.
Step 3.
Reviewing Your Plan
If you find during your plan that your needs have changed and your plan is not now meeting your requirements, we will assist you to complete documentation to lodge a change of circumstances form, collect and put together appropriate documentation and submit to the NDIS for consideration.
Prior to your plan ending we will meet with you and you can tell us if your goals are still relevant to your needs. We will contact your support providers to gather any reports or supporting documentation that will assist in the review process, to inform the NDIS of your support needs for the next 12 months.

SIL - Supported Accommodation
City to Country Care provides Supported Independent Living (SIL) which is help and/or supervision of daily tasks to help participants live as independently as possible, while building their skills.
City to Country Care provides this model of supported accommodation to people who want to feel included, to live life their way, be in a safe and happy home environment. You are able to have a place you can call home and live independently, where your choices and privacy are respected.
City to Country Care homes house up to 4 participants and have 24x7 carers on site who provide assistance and supports to you. You are able to enjoy the privacy of your own bedroom as well as shared spaces where you will have companionship with people who have similar interests as you do.
You will receive assistance and/or supports with daily living such as personal care, attending medical appointments, medication administration, preparation of meals, household tasks, transportation, community participation, assistance with positive behaviour supports and much more.
You will also enjoy a variety of supervised indoor and outdoor activities from shopping to movie nights, gardening in the backyard and BBQ’s with others.
What are the Benefits of Supported Accommodation with City to Country Care

Medium Term Accommodation
City to Country Care provide medium term accommodation as one of our home and living supports. NDIS provides this funding for somewhere to live if you can’t move into your long-term home because your disability supports aren’t ready. Medium Term Accommodation is generally funded for up to 90 days.
For example, you might need medium term accommodation while you wait for your home modifications to be completed. Or you’ve already secured a long term home, such as Specialist disability Accommodation, but you’re waiting for another person to move out before you can move in.
Medium Term Accommodation includes:
· 24/7 hour supports with all aspects of your daily care and activities in a fully furnished, self-contained, accessible premise
· All meals are provided throughout the day including breakfast, lunch, dinner, morning and afternoon tea and supper
· Activities that are fun which you and City to Country Care agree to ie lunches, football, movies, going to the beach etc.
Short Term Accommodation (Respite)
What is Short Term Accommodation
This is funding for supports and accommodation for you to live a short time away from your usual home. Sometimes, your carer may need a break from their caring role. Short Term Accommodation funding covers the costs of your care in another place. This can be used in a block of up to 14 days at a time or for one weekend a month. Maximum funding per plan is 28 days. You may have a short stay with other people, or by yourself.
Sometimes a short stay away from home gives you the chance to try new things; it can be a place to make new friends or develop new skills.
Short term accommodation includes:
· 24/7 hours staffed, fully furnished, self-contained, accessible house
· Personal care
· Accommodation
· Food – All meals provided throughout the day including breakfast, lunch, dinner, morning and afternoon tea and supper
· Activities that are fun which you and City to Country Care agree to ie lunches, football, movies, going to the beach etc.
ILO – Independent Living Options
What is ILO?
An individualised living option helps you use your funded supports to live the way that suits you. It can include things like personal care, help to build your skills, or support with household tasks like shopping or cooking. You can choose the home you live in and set up supports the way that best suits you. An individualised living option is the package of supports that can help you live how you want in the home environment you have chosen.
Having individualised living option supports in your plan will help you work out how you want to live, where you want to live and who you live with. There are lots of choices with individualised living options. You can share your home with friends or housemates. Or you might choose to live in one of City to Country Care’s ILO premises, or on your own with the support you need.
Individualised living option supports don’t include support for activities outside your home, such as work or study, playing sport or going out. If you need support for these activities, we’ll include it in other parts of your plan.
What is the Aim?
City to Country Care can support you to participate in community activities that you enjoy but you may not be able to participate in independently. Our support workers will be able to assist you in joining group activities in the community and ensuring you get there and back home safely. They can also provide you with company to do something you enjoy like going to the movies or out for lunch. This service will help you towards achieving your goal of increasing your social and community participation and possibly form new relationships in your local community with like minded people.
City to Country Care can assist you to access and participate in the community by guiding, mentoring, supporting and motivating you to be successful in achieving your goals and aspirations. This could be by going to the local café to have a coffee, visiting local parks, art galleries, museums, going to the movies, bowling, to the beach, on a ferry ride and much more.
Individual and/or Group Based Activities
City to Country Care is committed to helping you with social supports as this will help you to maintain friendships and keep health and active.
City to Country Care hold a range of activities for people with disabilities from movies to lunches, visits to theatres, gardens, museums and day trips and annual events. Whether you want to go in a group or just with a close friend or support worker, we are here to help you. Getting out and about will help you learn new skills such as how to confidently use public transport, see new sights, make new friends and most of all do the things you love.
What is a Psychosocial Disability?
The NDIS Commission defines psychosocial disability as:
‘A mental health issue that affects people’s daily activities such as socialising or interacting with others in a social setting, learning or self-care, or their capacity to fully participate in society’.
Some of the specific disorders include:
· schizophrenia
· obsessive-compulsive disorder (OCD)
· bi-polar disorder
· severe and persistent anxiety and depression
How Can City to Country Care Support a person who has a Psychosocial Disability?
The NDIA defines recovery as ‘achieving an optimal state of personal, social and emotional wellbeing, as defined by each individual, whilst living with or recovering from a mental health condition.
A psychosocial Recovery Coach is a new NDIS support for participants who require support managing complex challenges of daily living.
Recovery coaching is about empowerment and independence, increasing social and community participation. City to Country Care’s Psychosocial Recovery Coaches will work with you and your stakeholders to help you to take more control of your lives and to better manage the challenges you have with your day-to-day living. They have extensive qualifications and lived experience and provide supports in a holistic person centred approach. They will work collaboratively with you to develop and implement a plan, as well as assist with the coordination of NDIS and other supports you may require.

City to Country Care’s In-home support service assist people with a disability to live independently at home and in your community. We also provide drop-in supports to people who live independently at home. Our support staff will work with you to develop a plan of supports that are tailored to your needs, your plan will be reviewed regularly to make sure it is meeting any changing needs that can occur. This includes everything from helping people understand the disability care system to connecting you with services so you can live independently in your own home and will not be at risk of inappropriate placement in a 24-hour care facility.
City to Country Care provide the following in-home and drop-in supports:
Capacity Building Supports are designed to help build your skills and independence in your daily life. The funds are used to support goals that specifically build on your long-term independence. For example, Capacity building supports will provide you with the knowledge, skills and education to prepare healthy meals yourself rather than having meals delivered on a weekly basis, which is covered under Core Supports.
There are a number of Capacity Building areas that City to Country Care would be happy to support you with. They are:
v Support Coordination is a fixed amount of funding that can be used to engage a Support Coordinator who can help you to connect with, and implement your plan.
v Improved Living Arrangements helps you to find and maintain appropriate accommodation for your needs.
v Increased Social and Community Participation helps you with the development and training of life skills to encourage your participation in your community, and with social and recreational activities.
v Finding and Keeping a Job helps you to find and maintain employment. This funding can be used for employment related support, training and assessments.
v Improved Relationships helps you to develop positive behavioural strategies to better your interactions with people in your life and community.
v Improved Health and Wellbeing helps you to manage the impact of your disability through exercise and diet advice. It’s important to know, that gym memberships are not included in your funding.
v Improved Learning provides training and advice to help you move from school into higher education, like university or TAFE.
v Improved Choices helps you to manage your plan funding with the help of a Plan Manager.
v Improved Daily Living helps to build your capacity to engage within your community or to be more independent at home by providing assessment, training or therapy to help increase your skills.
City to Country Care’s Day Program and Group Activities provide a great way to meet people, make friends with others who have similar interests to you, developing life skills and connecting with your community.

What is the NDIS?
The National Disability Insurance Scheme (NDIS) was established by the Australian Government to give people with disability a greater choice and control over the support they receive.
